Terms for umbilical cord blood donation

Before you can donate umbilical cord blood you have to fill in a form (this from is sent to you via email). Furthermore, you must agree to the statement below:

• I have read and understood the brochure “Donation of umbilical cord blood”. I have been able to ask questions to the obstetric caregiver/midwife or the Sanquin Umbilical Cord Blood Bank, and my questions have been satisfactorily answered.
• I voluntarily give permission to the Sanquin Umbilical Cord Blood Bank to collect umbilical cord blood and make it available for the treatment of patients. I understand that by doing so, I cannot claim the product for myself.
• I agree that the blood taken during delivery and the umbilical cord blood will be tested for (infectious) diseases relevant to the receiving patient.
• I understand that Sanquin is legally obliged to inform the GGD if a notifiable infectious disease such as hepatitis (jaundice) is found in the blood.
• I agree to be notified if any abnormalities are found during the examination or blood test that may affect my health or the health of my child.
• I agree that my and my child’s data and blood samples will be stored indefinitely.
• I declare that all the information provided by me is true to the best of my knowledge.
