You can donate cord blood if you give birth at one of the following locations: the Bravis Hospital in Bergen op Zoom, the Reinier de Graaf Hospital and associated practices in Delft, or at OLVG East in Amsterdam.

Good to know: It is not possible to donate cord blood at other hospitals or at home births.

Submit your questionnaire as soon as possible

Before you can donate cord blood we will need your consent. To give your consent it is important that you read the brochure and fill in the questionnaire below. Hand your questionnaire as soon as possible in at one of the following locations:

  • the gynaecology outpatient clinic of the hospitals mentioned above
  • your midwifery practice
  • the delivery rooms of the hospitals mentioned above when you go into labour

Please note that this questionnaire has to be printed. It is not yet possible to complete the questionnaire digitally.

 

Are you unable to print the questionnaire?

If you are unable to print the questionnaire, a paper version is available at the locations mentioned above.

Good to know: You may complete the form up until the start of labour, but not once contractions have begun.

We will take care of the rest

All necessary materials are available in the hospitals, and transport of the cord blood to the Cord Blood Bank is arranged by Sanquin.

If the collected cord blood contains enough stem cells, it is processed into a stem cell product. Once processed into a stem cell product, it can be used for the treatment of patients with life‑threatening diseases such as leukaemia.
